What is DHT?

December 15, 2025|Dr. Shiwei Han

DHT is a hormone derived from testosterone through a conversion process facilitated by the enzyme 5-alpha reductase. While DHT plays crucial roles in male development during puberty, it can also contribute to hair loss later in life, particularly in individuals who are genetically predisposed to hair loss.

How DHT Affects Hair Follicles

The hair loss process involving DHT tends to progress in four distinct stages:

  1. The growth (anagen) phase of the hair cycle progressively shortens
  2. The resting (telogen) phase lengthens
  3. Hair strands become progressively finer with each cycle
  4. Eventually, the follicle stops producing new hair altogether

Interestingly, the visible effects of the scalp and back of the scalp remain lower DHT sensitivity, which is why pattern baldness typically affects the top and crown of the head while sparing the back and sides.

The Genetic Connection

Not everyone who has normal or even elevated DHT levels will experience hair loss. If you have inherited hair follicles with increased DHT receptors, you'll likely experience male pattern hair follicles and more progressive hair loss. These receptors are what allow DHT to bind to the follicle and begin the miniaturization process.

DHT Blockers as Treatment

Counteracting DHT's role is one of the most effective treatments that target this hormone:

  1. Finasteride works by inhibiting the 5-alpha reductase enzyme, reducing DHT production
  2. Topical treatments like DHT-blocking shampoos
  3. Natural DHT blockers such as saw palmetto and pumpkin seed oil, reducing DHT by up to 30%
  4. Topical anti-androgens to target finasteride and other treatments directly to the scalp

These medications can slow or halt progressive hair loss and often stimulate some regrowth, particularly when started early in the hair loss process.

Beyond DHT: Other Factors

While DHT is a primary driver of androgenetic alopecia, other factors can contribute to hair loss:

  1. Inflammation in the scalp
  2. Poor blood circulation
  3. Nutritional deficiencies
  4. Stress
  5. Medical conditions

This is why a comprehensive approach to hair loss often yields the best results.

When to Seek Professional Help

If you're experiencing progressive hair thinning or recession, consulting with a hair specialist is recommended. At Hairthetics, we perform comprehensive assessments to determine the cause of your hair loss and develop personalized treatment plans that may include:

  1. Medical treatments for DHT and hormone control
  2. Low-level laser therapy
  3. Scalp micro-pigmentation
  4. Surgical options when appropriate

Understanding the science behind hair loss is the first step toward effectively addressing it. With today's advanced treatments, hair loss is increasingly becoming a choice rather than a destiny.
